Physical Information

All students have three requirements to be able to participate in athletics at Heritage Hills High School/Middle School for the 2025/2026 School Year:

1. Final Forms Log-In 

   1. Only needs to be done once per household

   2. How to Register  (English)   (Spanish)

2. Final Forms Documents 

   1. First year form completion with yearly review/update as needed

   2. Final Forms Documents

   3. Print the "IHSAA PPE Phyiscal Form" to take with you to the provider 

   4. How to Switch to New Year (2026-27)

3. Pre-Participation Physical dated on or after April 1, 2025 

   1. New physical required for each school year

   2. Blank Physical Evaluation Form

   3. Upload completed Physical to FinalForms account.

*Athletes will not be allowed to tryout if the both the physical and Final Forms Documents have not been completed. Once the physical and Final Forms Documents are completed, your child may try out for one or multiple sports teams at Heritage Hills High/Middle School.
